External photograph of a 4-month-old boy with left unilateral coronal synostosis (UCS) showing ipsilateral retrusion of the forehead and elevation of the superior orbital rim with widening of the interpalpebral fissure (the “harlequin eye”; black arrow) with antero-inferior positioning of the ipsilateral ear. The side opposite the synostosis has compensatory bossing of the forehead and narrowing of the interpalpebral fissure (white arrow). The tip of the nose and the chin point away from the synostotic side (arrowhead).
