Cerebral angiogram, left ICA, AP projection pre (left) and post (right) transarterial balloon-assisted coil embolization. The patient presented with pulsatile tinnitus and left abducens nerve palsy six weeks after trauma. Angiography shows the Type A CCF with reflux inferiorly and contralaterally involving bilateral cavernous sinuses (black arrows). There is also evidence of steal phenomenon with decreased flow into the middle cerebral artery (red) territory and no visualization of the anterior cerebral artery territory.
