A 65-year-old woman presented with a case of IIH displaying symptoms of chronic headaches, transient visual loss upon postural changes (typically lasting seconds), and pulsatile tinnitus. Examinations revealed several key findings: bilateral papilledema (A and B), evident optic nerve head swelling in an OCT cross-section, notably more pronounced swelling in the nasal borders (C and D), an enlarged blind spot observed in the visual field (E and F), and an empty sella seen in the sagittal view of MRI (G). Additionally, MRA/MRV showcased the absence of signal in the right transverse vein sinus (H), contrasting with a clear signal in the left side (highlighted by a red arrow). Further observations included enhancement (with gadolinium) of the prelaminar optic nerve, distension of the perioptic subarachnoid space, and tortuosity of the orbital optic nerve (I), along with intraocular protrusion of the prelaminar optic nerve (J).
